  • An initial Video Visit or Home Visit is needed for all New Patients to establish with the practice. This allows a more personal encounter. Cost $80 New patients please call 305-290-1144 or email 4healthcalls@gmail.com to get started.

    Video visits are also available for Established Patients: where face to face is indicated to assist with proper diagnosis; such as more extensive skin rash, low back pain, joint pain, mild abdominal discomfort etc. or if this is the preferred visit type. Cost $60

    Video visits are conducted via Zoom; please download the app. https://zoom.us/ to your computer or smart phone or tablet.

  • Phone visits are for established patients specifically for non-complex care, for example, medication refill, known exposure to illness such as flu or Strep throat, follow up for recently treated illness, or review of labs etc.

    Texting is acceptable for non-complex visits, based on the concern, a photo of the throat, previously prescribed medication etc. is requested based on the health concern. Phone Visit Cost: $45.

    If during the conversation the concern is more complex, a video visit or home visit may be recommended for proper care.

  • For New patients and available to Established patients who are residents or visitors to Naples or Marco Island.

    An in-person visit is based on need, for example; physical limitation, in home testing; Strep, COVID or urinary symptoms, procedures such as ear lavage/wax removal, select injectable medications.

    New Patient Visit Cost: $150-250 Established Visit Cost: $100-200

    Visit Cost is based on distance traveled and visit complexity.

New Patients

An initial Video Visit is recommended, to establish care and set up your patient portal. For residents or visitors to Naples/Marco Island a home visit is an alternative.

  1. Please Call (305)-290-1144 or email 4healthcalls@gail.com to discuss your consultation needs, and to provide your an email address.

  2. We will email you a link to access the patient portal and set up your private patient account.

  3. Please set up your patient profile by completing the demographic information and medical history.

  4. Upload a drivers license or state ID in your profile. In the upper right corner, place cursor over the icon above your name, click manage account, then edit (pencil icon) and upload your photo ID.

  5. Select a New Patient video visit or home visit and schedule a time.

  6. Please enter your account as self pay and key in your credit card information. All credit information is encrypted via Bluefin (the leader in payment security)

  7. Please complete and submit the New Patient Intake questionnaire along with a signed consent.

  8. Ready to start your visit!

Established (returning) Patients

  1. Login to the patient portal www.charmphr.com.

  2. Select the visit type which best suits your need and schedule an appointment.

  3. You will receive a confirmation email and a separate email link to complete a brief questionnaire specifying the reason for your visit and make any changes to your medications or allergies since your last visit.

Video Visits for established patients are best for:

A health concern which would benefit from a face-to-face evaluation such as: more extensive skin conditions, swelling of a joint, low back pain etc.

Which Types of Visits Are Perfect for Virtual Care?

Follow Up Visits

Once a patient has been seen for a condition, such as a minor infection or injury, follow up visits might be appropriate to ensure that the patient is healing as expected over time. The follow up gives the patient the opportunity to report their progress and the provider a chance to provide additional recommendations. For many conditions, these visits can be conducted virtually.

Medication Management

Many patients are on medicines prescribed by their primary provider, however because of travel, extended trips or misplaced medication, a short course of a medication will provide the proper medical management. No physical exam is usually necessary, making a video visit using telemedicine technology ideal. Simply provide the previous dispensing pharmacy information and upload a picture or the prescription bottle to confirm medication and dosing.

Chronic Illness Care

When an appointment with a primary provider is not timely or accessible due to travel etc., treatment for previously diagnosed illnesses such as Gout flare, mild Asthma exacerbation, GERD (gastric esophageal reflux), Low Back Strain, Acne, Urinary Infections etc., may be done virtually. This reduces the burden on already ill patients and improves health outcomes.

Sharing Test Results

Rather than scheduling a visit to share test results or losing the personal touch by doing it over the phone, telehealth provides a simple, convenient way to communicate results and make recommendations in a timely manner.

Flu, Cold, or Allergy

Instead of exposure to other illnesses in a waiting room, schedule a video visit. In most cases, flu, cold, sinusitis, sore throat and allergy appointments can be handled over video–including prescribing follow-up care. A consultation may also determine if a physical exam or other form of medical care such as diagnostic studies are indicated.
