PAYMENT

Currently, credit card payment for online or phone visits is due at the time of your appointment. Cash or credit card payments are accepted for in home visits.

Rest assured, our practice does not process credit card information directly. ALL credit card information is maintained via Bluefin, the leader in payment security.

A visit summary is available on the patient portal in your patient profile. This summary will contain the diagnosis code(s), care provided and treatment plan, to submit to your insurance for possible reimbursement.

We understand circumstances change. Cancelled appointments are not charged at this time, however, please call 305-290-1144 or email 4healthcalls@gmail.com to cancel and reschedule.

NOTE: Medicare reimbursement for home visits is based on predetermined criteria according to medical necessity.
